GLP-1 Medications: Benefits Beyond Weight Loss

GLP-1 medications have become widely known for their role in weight management, but their effects may extend beyond the number on the scale. Research on specific GLP-1 medications has demonstrated benefits related to blood sugar control, cardiovascular risk, and other measures of metabolic health. However, the evidence and approved indications vary by medication, so it is important not to assume that every benefit applies equally across the entire drug class.

What Are GLP-1 Medications?

GLP-1 receptor agonists mimic the effects of glucagon-like peptide-1, a naturally occurring hormone involved in blood sugar regulation and appetite. They can stimulate insulin release when glucose levels are elevated, reduce glucagon secretion, slow gastric emptying, and influence areas of the brain involved in appetite and satiety.

Several GLP-1 medications are used in the treatment of type 2 diabetes, and some are also approved for chronic weight management. While they share a common mechanism, individual medications differ in their indications, dosing, and available clinical evidence.

Cardiovascular Health

One of the most important areas of GLP-1 research involves cardiovascular health. In large clinical trials, certain GLP-1 medications have been shown to reduce the risk of major cardiovascular events in specific patient populations.

For example, the SELECT trial found that semaglutide reduced the risk of cardiovascular death, nonfatal heart attack, or nonfatal stroke in adults with overweight or obesity and established cardiovascular disease who did not have diabetes. These findings suggest that, for appropriately selected patients, the potential benefits of certain GLP-1 medications may extend beyond weight reduction alone.

Blood Sugar and Glycemic Control

GLP-1 medications were originally developed for the treatment of type 2 diabetes, and improving blood sugar control remains an important benefit of these medications. By enhancing glucose-dependent insulin release and reducing glucagon secretion, GLP-1 receptor agonists can help lower blood glucose levels and improve HbA1c in people with type 2 diabetes.

Some research has also examined their effects in people who do not yet have diabetes. In an analysis from the SELECT trial, participants with overweight or obesity and cardiovascular disease who received semaglutide were more likely to return to normal blood sugar levels and less likely to progress to diabetes compared with those receiving placebo.

Risk of Progression to Type 2 Diabetes

For people with overweight or obesity who are already at increased metabolic risk, changes in blood sugar over time can eventually lead to type 2 diabetes. Research with semaglutide suggests that treatment may influence that trajectory in certain populations.

In the SELECT trial, participants receiving semaglutide were less likely to progress to biochemical diabetes compared with those receiving placebo. These findings are promising, but they should be interpreted within the population studied and should not be taken to mean that every GLP-1 medication prevents diabetes.

GLP-1 Medications Aren’t Right for Everyone

Although GLP-1 medications can offer meaningful benefits for appropriately selected patients, they are not the right choice for everyone. Like all medications, they can cause side effects and have potential risks that should be considered alongside the expected benefits.

Common side effects include nausea, vomiting, diarrhea, constipation, and abdominal discomfort. Individual health history, current medications, treatment goals, and potential contraindications should all be considered when determining whether a GLP-1 medication may be appropriate.

The Bigger Picture

GLP-1 medications can be powerful tools, but medication is only one part of long-term metabolic health. Nutrition, physical activity, adequate protein intake, resistance training, sleep, and other lifestyle factors remain important for supporting overall health and preserving lean mass during weight loss.

The goal is not simply to see a lower number on the scale. A more complete approach considers body composition, metabolic health, cardiovascular risk, and the habits that can support health over time.
